Job Description:
We are seeking a dedicated Home Health LVN/LPN to provide compassionate care to patients in their homes. This role requires expertise in home health and proficiency in HCHB/OASIS documentation.
Responsibilities:
• Deliver quality nursing care to home health patients.
• Complete same•day documentation using HCHB.
• Manage 20•25 cases weekly, achieving 5•7 productivity points daily.
• Attend weekly in•person office meetings.
• Be flexible to float as needed.
Requirements:
• Valid LPN License and BLS certification.
• 2+ years of home health experience.
• Proficient in HCHB/OASIS, including Oasis E post•January 2023.
• Ability to work 40 hours weekly, no overtime or on•call.
• Reside within 30 minutes of the territory or secure nearby housing.
Additional Information:
• 13•week contract with extension potential.
• 2•3 days of orientation.
• Flexible branch hours: 8am • 5pm.
